Household consent addendum
This addendum authorizes LeakTrace Cyber Intelligence to include the individual named below in the intelligence coverage of a family office or principal engagement. One addendum should be completed per covered household member.
1. Scope of monitoring
By signing below, the individual named authorizes LeakTrace to conduct continuous intelligence coverage of the following:
- Public exposure of the individual's email address(es) in monitored breach databases
- Public mentions of the individual's name in news, search indexes, and social platforms
- Public reference to the individual in dark-web or non-public sources monitored by LeakTrace
- Reputation signal analysis of coverage involving the individual
2. What LeakTrace does not do
- Access the individual's mailbox, devices, financial accounts, or any password-protected system
- Contact adversaries, adversary infrastructure, or the individual's known associates without prior separate authorization
- Publish or share findings involving the individual with any party outside the engaged family office or principal contact
- Retain findings after the individual withdraws consent, beyond the operational archive window described in the Terms of Engagement
3. Data retention and withdrawal
Findings surfaced under this coverage are retained per the LeakTrace Terms of Engagement. The individual may withdraw consent at any time by written notice to [email protected]. LeakTrace will confirm withdrawal within one business day and cease active monitoring of the individual within five business days. Historical findings will be archived and access to them restricted per standard retention.
4. For minors
Where the covered individual is under the age of majority in their jurisdiction, this addendum must be signed by a parent or legal guardian. LeakTrace does not conduct social platform monitoring or reputation analysis on minors under age 13.
5. Acknowledgement
By signing below, the individual (or their legal guardian) confirms that they have read and understood this addendum, that they have received and reviewed the LeakTrace Terms of Engagement, and that they authorize the scope of monitoring described above.
Completed addenda should be sent to the engaged principal's Chief of Staff or forwarded directly to [email protected]. Retention: the executed addendum is stored under the associated ClientAccount for the duration of the engagement.
